The Horizon Health Senior Care team provides high-quality assistance for seniors dealing with life’s many challenges. Our medical needs increase as we get older, and it’s important to have a medical team that is familiar with these needs and will support you or your loved one. Your first visit to Senior Care will consist of a physical and mental health evaluation, medical history review, and discussion of treatment options going forward.
